Your hormones were never designed for aviation

One thing I genuinely struggle to understand in aviation is how normalised menstrual disruption has become for female crew.
Cabin crew is a predominantly female profession. And yet… we teach almost nothing about how the aviation lifestyle can affect the menstrual cycle.
We teach cabin crew how the cabin environment affects the body. But conversations around female hormonal health are still almost invisible in crew training. And, as someone who worked in crew training myself… I find that quite difficult to understand.
Because this is not some niche “wellness” topic. This is basic human physiology.

The menstrual cycle is deeply connected to sleep, stress, the nervous system and the body clock. Yet many women in aviation spend years trying to “push through” symptoms without ever understanding what may actually be happening physiologically.
Hormonal regulation depends on constant communication between the brain, circadian rhythm, metabolism, light exposure and stress signals throughout the body. Which means the menstrual cycle is not separate from sleep, fatigue and operational stress. They are all connected.
At the centre of this communication is something called the Hypothalamic–Pituitary–Ovarian axis, often shortened to the HPO axis. Very simply explained: the brain sends hormonal signals, the ovaries respond, hormones rise and fall in rhythm, ovulation and menstruation happen through coordinated timing.
And timing is the important word here, because the female reproductive system relies heavily on biological timing signals.
The body likes rhythm.
Predictability.
Consistency.
Light and darkness.
Sleep and wakefulness.
Food timing.
Stress patterns.
Body temperature rhythms.
All of these help regulate the body clock. And the body clock influences much more than sleep.
It also helps regulate cortisol, melatonin, reproductive hormone release, appetite, glucose regulation, energy levels, body temperature and nervous system activity.
Which means your menstrual cycle is not separate from sleep and circadian rhythm.

Now aviation enters the chat.
Night flights.
Jet lag.
Time-zone crossings
Sleeping at biologically wrong times.
Early wake-ups after a broken sleep.
Inconsistent meal timing.
Fatigue.
Operational stress.
The body starts losing predictable timing cues.

And this is where things start making a lot more sense.
Research suggests that circadian disruption and shift work may interfere with hormonal regulation and menstrual regularity.
A 2023 meta-analysis involving more than 195,000 women found that shift work was associated with:

  • higher odds of irregular menstrual cycles

  • more painful periods

  • and earlier menopause

In simple terms, women working irregular schedules were more likely to experience menstrual disruption compared with women working regular daytime hours.

This does not mean aviation “causes” every hormonal issue. Human physiology is far more complex than that. But it does show something important:
The environment female crew work in contains several factors already known to affect hormonal regulation. And aviation adds another layer on top of that - physical load.
A 2024 study looking specifically at flight attendants found that physically demanding tasks such as lifting, pushing, pulling, bending and prolonged standing were associated with more frequent or more painful menstrual symptoms and irregular cycles.
And this is exactly why I think we need to start talking about this.
Many women in aviation spend years thinking something is wrong with them. That they are “bad at coping.”
Weak.
Emotional.
Lacking energy.
Lazy.
Overly sensitive.

When in fact, the body may simply be responding to an environment that constantly interrupts its normal biological rhythm. Your hormones rely heavily on timing signals, and aviation constantly interrupts those signals. And maybe understanding that changes the conversation.
Not in the direction of making women in aviation seem fragile.
It is about finally acknowledging that female physiology exists inside this industry, too.
And the more we understand what this lifestyle may be doing to the body, the better we can support the people living it.
